Lifeplan has been providing disability support services to the Perth community for nearly 20 years. It’s our goal to support you in your day-to-day life so that you feel safe and comfortable in both your home and within your community.
Daily Living Support Services
Being committed to the well-being of the people we support means that we aim at providing a reliable and respectful service to ensure that your daily living needs are met. Our daily living support services are tailored to meet everyone’s daily needs and includes but is not limited to helping you get up in the morning, giving you support with personal care such as showering, getting you dressed and much more.
Recreation & Leisure Disability Activities
At Lifeplan, we believe that getting out and about as well as doing activities can help you improve your physical health and gives you a greater level of independence and freedom, both of which are necessary to enjoy life to its fullest. Whether you want to be part of something new, or want to continue your current activities, Lifeplan is here to help you engage with your local community with our recreation and leisure disability activities.
Independent Living Support Services
We passionately provide support to people with disabilities who would like to shape their lives more independently. With a range of tailored independent living programs, we can help you manage personal care, keeping your house tidy, providing tenancy advice and much more.
Peer Support & Mentoring
We believe that people who have dealt with similar experiences in their lives like yours, are best suited to support and mentor you along your individual life journey. These lived experiences by mentors range from a variety of circumstances, including mental health, indigenous matters, drug and alcohol, youth problems, LGBTQI+, immigration and refugee experiences. Lifeplan can assist sourcing a mentor at your request to help get you to the next phase in your life plan.
Mental Health Services
According to the World Health Organisation (WHO), 1 in 4 people in the world are affected by mental or neurological disorders at some point in their lives. It shows that you are not alone with your predicament. At Lifeplan we not only promote an open discussion about this stigmatised topic, but we also offer active support for people affected. Our main goal is to help you get back on track in your life.
