Lifeplan's Approach
At Lifeplan, we specialise in one-to-one supports for people with disabilities throughout Perth, Western Australia. We are driven by our customers to deliver a service that is tailor-made to meet the needs of the individual. Working closely with customers and their families, local planners and coordinators, Lifeplan provides flexible supports both in-home and in the community to assist people to:
- Connect with their self-identified community
- Engage in personal development, and pursue wellbeing
- Pursue recreational activities of their choice
- Engage in independent living skill support and training
- Develop work readiness skills
- Access respite and take a break
Lifeplan understands the need to ensure that our supports develop and adapt to the needs of our customers, and to remain meaningful and purposeful in our approach at all times.
Lifeplan has a fleet of vehicles, both modified for wheelchair access and standard vehicles designed to even further personalise supports.

GUARDIANS, LOCAL COORDINATORS, AND PLANNERS
Lifeplan recognises that sometimes other people are an important part of your decision-making process. We understand our part in this decision-making process and work closely and openly with Guardians, Local Coordinators, and Planners to facilitate the positive outcomes of your decisions and choices.
