Here at Lifeplan, we are committed to helping you live your life, your way. We do this by providing tailored, individual supports, to suit your lifestyle. Part of this process, is ensuring you have an NDIS Plan in place to meet these needs. An important part of getting this plan in place, is clearly defining your life goals to your NDIS Planner. In other words, defining what you need to live your life, your way.
Before your NDIS Planning Meeting, you should have a clear statement of goals and aspirations. This is necessary as part of your NDIS application, and its vital to ensure you receive the right support services. Your NDIS Planner will ask about your goals and what you want to achieve, and they will use this information to help provide the right funds to help you achieve these goals. This is why it’s so important to get these goals right, so you receive the right supports.
Goals
What do we mean by your life goals? As simple as this sounds, you need to clearly define how you want to live your life, so the NDIS Planner can put the correct funding processes in place. Chat to your family and friends about what your goals are, as they can help you put this together (you might not think of something that is routine for you every day, so they can help point out anything you might overlook).
We understand it can sometimes be hard to put your life plan down on paper, so don’t worry, we are here to help! Lifeplan can provide tips and suggestions to make sure you include everything you need to live your best life. We are also happy to attend your planning meeting with you, to make sure your goals are well represented to the NDIS planner.
What are your life goals? We have put together some questions to ask yourself, to start thinking about the right goals to achieve your life plan:
Where do you want to live?
Do you want to live independently, or in shared accommodation? Independent and Daily Living supports can be any number of supports such as help getting ready in the morning, assistance in paying bills and dealing with landlords, medication prompts, cooking and meal prep, the list is endless. Once you have defined how you want to live, your NDIS planner can ensure you have the right funding in place to help you achieve this goal.
Do you have / do you want a job?
Having a job is a great way to stay connected to the community and maintain your independence. It’s obviously very important you include this as a goal, so your planner can ensure you have the tools you need to help you find employment and the support you need to maintain this. This might be anything from skills training, transport and tools to maintain your employment.
Do you want to get out and about in the community?
We can help you connect with people in your self-identified community, or help you maintain your current recreation and leisure activities. This can be anything from attending sporting events, participating in hobbies, group activities, visiting friends and family or exploring the city of Perth. Ensure you include any activities in your goals, so you can get the right funding to participate.
Do you want to improve your health and wellbeing?
Your mental and physical health are a huge priority, so you need to include in your goals, what you need to maintain or improve your health and wellbeing. This can be anything from mental or physical therapy appointments, maintaining a balanced diet or help with food cooking and preparation, recreation and exercise.
Is there equipment or modification that would help with your life plan?
Part of the NDIS funding covers payments for capital budget. This includes a range of assistive technologies, equipment, home or vehicle modifications, and funding for capital costs you may need.
Do you need help getting out and about?
How are you getting to your job, activity, meeting, training? Make sure to include transport in your goals, and whether you require any specialised vehicles to get you where you need to go.
NDIS Goal Setting is vitally important, to ensure you receive the correct funding to live your life, your way. At Lifeplan, we are happy to provide assistance in any way we can, so please feel free to contact us to discuss your individual needs.
